1. What’s the purpose of this form?
According to the IRS, Energy Trust commercial and industrial program incentive payments are subject to 1099 reporting. A W-9 is required to complete the 1099 at year-end. Energy Trust will accept the IRS Form W-9 or this Substitute W-9. Energy Trust created the Substitute W-9 as an alternative format to collect the needed taxpayer identification information. Energy Trust hopes that the Substitute W9 format will help us more easily identify the legal name of the business that we are providing with an incentive.
2. Why does Energy Trust request participant tax identification information?
Energy Trust is required by law to track and report all commercial incentive payments to the IRS at the end of the year. Energy Trust does not give tax advice, and recommends that you consult with your own legal and accounting professionals to determine whether or not you are required to report an Energy Trust incentive payment to the IRS at tax time.
3. How do I fill it out?
Please follow the directions on the form and fill out only one Type of Entity section (A, B, C or D) depending on your business type.
4. Who can answer my questions?
Contact the Production Efficiency program at production@energytrust.org or 503-445-7643.

consumption is everybody’s concern. It’s also good
business sense, given that energy is a significant
component of operating costs, and improvements
can yield immediate results. The incentives from
Energy Trust were a big factor in helping us achieve
